We currently have a 50 user Server based Confluence. Not everyone will need to use this plugin. Would we need to pay for a 50 user version of this plugin or can we assign certain users to have access?

It's not quite clear what plugin you are referring to. But anyway, commercial add-ons on the Marketplace follow the user tier of the product you purchase them for. If your Confluence instance is 50 user, than so is your add-on.
