Need help embedding issue collector into Confluenct

I created an issue collector and am trying to embed it on a page in Confluence. Here is the message that I am getting and I don't know how to fix it. Can anyone please help.

Thank you Manoj, but this is a bit too much for me. I was hoping someone can just tell me what to do exactly.

Are you putting the code inside Html macro or using some other macro?

Hi, yes adding code inside HTML macro and now when I hit Update it just shows a blank page, where before it showed that error message.
