There's no insert row/column button on the spreadsheet macro as far as I can tell.

Not sure what app are you using exactly, but our Table Filter, Charts & Spreadsheets for Confluence app provides similar Table Spreadsheet macro.
So, right click your mouse and select the required option from the menu:

Could you specify what app you are using ? If it’s Elements Spreadsheet for Confluence you can insert row/column directly from the toolbar:
Or, right click your mouse and select ‘insert’ >> ‘insert row’.
