How can I add the Q&A functionality without having access to the Questions macro?

You could use the expand macro to create your own Q&As within confluence.
I actually want the reader to be able to submit a question and then display the response once the question is answered.

Oh. In that case, you should know that Confluence can't do that out of the box. You should use an app for this.
