I have an intake form that emails responses, I need a linked response log that feeds from the form.

We have  created an intake form on Confluence to open new cases. Once the respondent hits submit their intake form is emailed to multiple people. I need this same information to populate into a response log style form so we can have all answers and issues auto populated to one central place. This is likely a simple solution. I just don't know my next steps.
