How do you get the page you create to appear as a subpage of the one you're currently on?

This question is in reference to Atlassian Documentation: Create from Template Macro

I'm using the create from template macro and the only option available, regardless of which page the macro button appears on is to create a page that sits at the top level of the hierarchy under the space.

How do I use the button to create a sub-page of the page I am currently on?

When you create a page from a template by clicking create at the top of the screen, it creates the page as a sub-page so why not the same when using the macro?

That bug link states that it was fixed in 5.2.3. Have you tried upgrading?
