I have added a template and followed instructions as per https://support.atlassian.com/confluence-cloud/docs/promote-a-template/
But the template I created is not in the list. Please send further (details instructions on this) including who has authority to do this, as I have been provided with admin access, but note I cannot access Document Approval tab within the Space Settings.

Hi Cathy, If you are the space administrator you can create space templates and they will appear at the bottom of the template list unless you promote them. A space template will only appear in the space you created it. I tested on my Confluence cloud and it did promote my "short" list of templates to see first and it was listed within that group alphabetically.
Double check your settings in Global Admin.
Confluence System admins have rights to create Global Templates Note If you created a Global template you cannot promote it it will be at the end of the global template list.
Are you on a free instance of Confluence cloud? I cannot find the Document approval setting you are mentioned in your question
hello @cathy.c , a space admin can create a template
. After you create your template you can goto space settings --> content tools --> User created templates
Once you promote your newly created template it will be displayed on the top of your create dialog as shown in below screenshot.
