But we would like the workflow to be users initiate/self-serve. The idea is to embed a link that will direct user to complete a separate form with a list of questions at the time of the request. Is that possible?

@Kenny Ng ,  i dont think thats possible. There is a feature request for the same, you might want to vote on the same.

Yes, you can send personalised invite. Try the Personalise email invite option. Below page has the info like how can you get into the option.
