You might be able to achieve a simple version of this by Creating a template which supports variables for test, multi-line text, and suggested selections from a list.
You'd then want to pair that with the Create from Template Macro on your main parent page.
But for checkboxes, and more advanced selections, etc. you'd really need an add-on. There's several, but here's a few:

Well, the closest thing to a checkbox is the Confluence task. The problem for using it in a form is, of course, that it can also be unchecked and checked in read mode and thus also accidentally.
