Step one:
Check if your image is properly attached to the page in the Attachment menu:

In the example above, you see two files: PDF and JPG. The first item on the list is actually an image added from a URL.
Step 2:
Sometimes, when you're adding images (for example, copy pasting content from another site - Confluence space, website).
If you add an image from Confluence, and you don't have permissions to that image, it will not be displayed.
If you add an image from a website, you may end up with a file that's actually a hyperlink (when you click the image in the view mode, there will be a URL, not a file name, in the top left corner). If that link stops working, the image will not load.
(The above presumes that the permissions to add attachments are sorted out)
