Welcome to Atlassian community.
There's a Change Log template available in the Jira Report Blueprint. You'll have access to this Blueprint when Confluence has been connected to a Jira instance. For more information on this template, give the blog How to Document Releases and Share Release Notes a read.
If neither of these match your needs, you can also create your own template.
Lastly, there are some templating add-ons available from Atlassian Marketplace. You can trial these add-ons in case you want to see what sort of functionality they offer.

Thanks Srinath but Confluence is a horrible platform for maintaining a changelog. The more we use Atlassian products, the more we're disappointed with lack of features and painful upsells for targeted services (like StatusPage).
All of my team members dislike using Confluence; it doesn't work well with copy/paste from MS Word or Google Docs, the UI is confusing, and organizing documents is a sincere pain.
Right now, the only reason we continue using Atlassian is BitBucket and Jira.
