In order to properly help you, could you share a bit more detail what document and what approval you’re talking about?
Confluence by default does not know the concept of a document - only (mainly) spaces and pages. Are you using an app like Scroll Documents or Comala Document Management?
It also sounds like you can do this on the desktop - maybe you want to share a screenshot?
