Do we have the ability to build distribution groups by location, dept...etc?
What exactly do you want to use a "distribution group" for?
I think there are multiple ways to create group, the best option depends on what you want to do with them afterwards.
We want to be able to send out alerts based on various attributes such as location, department..etc.

I think to represent a distribution group, you should just create a regular Confluence group. Then you need a tool that can send alert to a whole Confluence group.
With the custom notifications feature in our Better Content Archiving app, you can periodically execute a structured CQL search, and if there is any page or blog post found, you can email them to a whole group using a customizable email template.
Here is how the configuration looks, it is quite self-explanatory. As you see you can select even multiple groups as recipients:
So, the bottom line is that if you can create a CQL search from your alert criteria, then it will just work!
