I've been working with Confluence for about 1.5 years and this is one of the most annoying things that wastes my time.
Often you need to update an image in a page without changing the caption.
There doesn't appear to be a way to do this. No menu option to 'update image'?
If you paste it overwrites the whole image and you have to type in the caption again.
Also, there is no way to paste text into the caption entry box. When I put the cursor in the caption entry box and press CTRL-v, the text ends up next to the image above the caption.
Is everyone else suffering this bug?

Yeah... This probably could be improved a bit. I've tried searching open suggestions on the topic, but couldn't find anything that fits. You could reach out to Atlassian Support and discuss this requirement with them, and their team can probably create a new suggestion on which you can vote and leave comments.
However, there is a workaround you could use:
Not the most intuitive workaround, but it should work.
