Is it free for government institutions?

I'm chief of it public health department, and we are using Bitbucket for only four of project unit. It'll still being free if we add more than 5 developer if I'm non commercial organization?

No. 

Secular charities and open source organisations can get free licences.  Academics can get a big discount.  That applies for the Atlassian stack in general.  Bitbucket Cloud is slightly different in that it also allows for up to 5 users of private repositories for free for any organisation.  But you'll need to pay if you fall outside of those rules.
