How do I attach a Project Template to a JIRA issue?

I have a Quality Checklist template created under 'Simple Tasklists tasks'. Need to attach this to the JIRA issue so that a reviewer can provide a signoff on test results. 

How do I attach this template to a JIRA issue?

Do want a template text to be created in your tasks or just a default attachment of the template attached to your task?

If it is a template text - you could follow this -

For the later - I would put a link in your Jira to a confluence page for them to review.

Maybe I am not understanding, could you not use the add attachment from the menu.
