How to create forms and surveys in Confluence Cloud

Looking to gather feedback from employees or customers without adding noise to your existing toolstack?

How to create a form in Confluence Cloud

 

Confluence doesn’t have a built-in survey tool, so you'll need an app to help you. Our tutorial uses Forms for Confluence, which lets you create surveys, quizzes, polls, and Jira task forms securely in your Confluence site.

  1. Click Apps > Forms for Confluence.

  2. On the Forms homepage, click Create new form.

  3. Enter your form name and choose where it will live. Add any collaborators, and configure advanced settings, such as email notifications (optional). Then click Continue.

  4. You'll be taken to your new page with an empty form. Click the edit icon (the pencil) on the form.

  5. Add fields to your form in the left-hand view of the editor. In the right-hand view, you can customise your question answers. 

  6. When you’re done, press Save.

  7. Publish or Update your page to see your completed form.

  8. Share your page with others, and they'll be able to respond to your form.

Just open your page in edit mode, type /form, and select Create Form from the dropdown menu.

How to view form responses in Confluence

 

Once you've created and shared your form, you can view responses from the Forms for Confluence homepage.

  1. Select Apps > Forms for Confluence in the sidebar.

  2. In the Forms for Confluence homepage, choose your form from the list and click its name.

  3. Choose a tab to view your response data:

    • In the Summary tab, you can view interactive data visualisations of each question.

    • In the Responses tab, click a respondent’s name to display their individual answers.

 

⚡Want to add a data visualisation to a report? 

 

In the Summary tab, hover over a visualisation and click the document icon to add it to a page or the download icon to download it as a PNG for external use.

How to display form responses on a Confluence page

How to embed a response table

  1. In edit mode on your Confluence page, start typing /responses table. Select Responses Table from the dropdown menu.

  2. Select your form from the dropdown menu.

  3. Choose whether to share the responses with everyone or make them available only to specific users.

  4. Click Embed responses, then Publish or Update your page.

 

How to embed data visualisations of your responses

  1. In edit mode on your Confluence page, start typing /visualisations. Select Embed Visualisations from the dropdown menu.

  2. Select your form from the dropdown menu.

  3. Choose whether to embed visualisations for all questions or specific ones only.

  4. Click Proceed, then Publish or Update your page.
