Step 1: Look for the week where the line changed

You’re looking for a visible change, such as:

  • QA / Validation time suddenly increasing in one week;
  • the trend slowly rising across several weeks;
  • one spike that makes the whole delivery picture look worse;
  • a week where Cycle Time increased and QA time increased at the same time.

This is the first useful signal.

If QA time was stable for several weeks and then jumped in one specific week, the issue probably isn’t “QA is always slow.”

It is more likely:

“Something about that week changed.”

That is a much better problem to investigate.

It is smaller. It is kinder. It is also harder to argue with.

Step 2: Click into the week that looks suspicious

A trend line is good.

A trend line with the actual work items behind it is better.

1. QA traffic jam

What you see:

Likely cause:

The team pushed too much work into QA at once.

  • move smaller pieces to QA earlier;
  • limit work entering QA near sprint end;

2. Rework loop

What you see:

  • QA / Validation time increases;
  • the same work items move back from QA to development;
  • bugs or stories are reopened.

Likely cause:

The team is discovering quality or requirement issues too late.

  • make acceptance criteria testable;

  • clarify edge cases before the work reaches validation.

  • only one or two work items caused most of the increase;
  • the rest of the team’s flow looks normal.

A large, complex, or risky work item skewed the weekly metric.

  • split similar work next time;
  • flag high-risk items earlier;
  • avoid treating one exceptional item as a team-wide performance issue.

Sometimes the workflow is not broken.

Step 5: Pick one improvement, not twelve

The point of a 15-minute bottleneck check is not to redesign the entire workflow before lunch.

Pick one improvement.

For example:

  • “No large stories enter QA on the final sprint day.”
  • “Every story needs clear acceptance criteria before development starts.”
  • “Reopened QA items are reviewed in standup.”
  • “QA-ready work should arrive throughout the sprint, not in one batch.”
  • “Large validation tasks must be split or flagged before the sprint starts.”
