Exportword.vm, attempted to include images in template, missing in word doc

I've been editing the exportword.vm template to match our companies look/feel for exported docuemnts, have successfully created a MSWord header/footer in css/html, so but am having trouble attaching a default logo to the header (or the body for that matter).  have tried just dumping some images to the logo directory and point to that: 

<img src="/images/logo/supplier.logo" alt="logo" />

That results in a blank image.  So followed the guidance from another how to page, and pasted my logo into another page, saved it, and got the id of the page from properties, ending up with: 

<img src="/download/attachments/42074119/supplier.png"/>

Again, no luck, image placeholder is in the resulting word doc, but is blank. The actual images in the exported doc are all present, its just the extra images that I want to place in the headers that are not linking/opening.  I've got a feeling that this open ticket may well be the cause:
