Automatically attach a form to a newly created ticket

We are using this tool for a large software implementation project. We have many projects with different requirements. We would like to be able to attach a form automatically on creation of a NEW Jira ticket per different project. Example is a Change Request form on our Change Management project.

 

Short term i am using automation to to add a comment with a share point location, but there must be a better solution.

Which product do you have? Or are you using for your Projects?

Forms are available, but what you're looking for is likely only in Jira Service Management natively.
