How to create a data entry form

Currently using Microsoft Access database to capture data on staff call outs (sick, latenesses, vacation). Need to replicate tool in JIRA starting with replicating the form. Hoping I can do with JIRA.

The "create issue" screen is a form. Work out what fields you want to report on, and add them to the "create issue" and probably "edit issue" screens.
