How can I create and transition a task after a custom drop-down option field is changed?

Good morning, I am currently trying to create a task once a drop-down Custom field has been selected.

 

I have 2 Custom fields, one being QMS Owner approval and the other being process owner approval. Both have the same options for the drop down either being approved or denied. These custom dropdowns are on multiple different tasks. and look as shown below. Capture12.PNG

The issue I am having is after both have been changed to either approved or denied the task needs to be transitioned and a new task will be created (both sharing the epic as the parent). At the moment I have all of the automation set up besides how to trigger the automation. I do not want to set up approvals sense there are multiple, and you can only set one approval per task. The automation I have so far looks as follows:

I have tried both approvals completed and When value changes. I have never delt with a transition like this so if anyone has any suggestions that would be greatly appreciated.

It is now recognizing the "when value changes" trigger, however, I am getting errors

Each if block is a separate variation of Process Owner approval equals approved or denied and QMS owner approval equals approved or denied
