Need help with empty field automation

I have an automation that sends me an email every time a 'fix version' is empty - however, it continues to send me emails when the fix version exists? Is this more of a time that it runs type of issue?

Example is one where the history doesn't match what's in the story (there is a present fix version) and I got an email that the fix version was missing 30 minutes ago (when the status transitioned)
