Is there a way to hide a form in jira issue?

Is there a way to hide a form inside the issue, or restrict to only certain people can see the form while others can't see it?

Not that I know of; the Form is visible to all users as an extension of the issue itself.

What is the need here? Why are you looking to hide the form?

I want to create a form of each department (HR, Cyber Security, IT...), i don't want HR to view other departments form, only their form. The same scenario applies to the rest of the department. 

If it doesn't work with form, can I use subtasks of each department, and create a privacy?

If this is a Company-managed Project, you can hide Sub-tasks per department.

You can use an Issue Security Scheme to do this:

• Create a Scheme, and assign it to your Project
• Create a Security Level per department, with a method to identify each department's users - eg. a Group, a Project Role, etc
• Assign the Security Level as each Sub-task is created
  • You can do this using Automation if there's a method to identify the department in the Sub-task
