Jira cloud comes with REST API and the ability to create issues. Having said that, the only thing that you should look into, is HOW to send from MS Forms these http requests. E.g. using the

POST /rest/api/3/issue

you could create issues in Jira. However you should find a way to send this request from MS forms. Do MS Forms have any sort of automation and the ability to send http requests?
