I have 3 users that have the exact same rule in Notifications to Add them as a watcher when an issue is created.
All 3 are being added as watchers. 2 are getting email notifications, but one is not.
It looks like that user's settings to receive notifications is correct.
You note an automation rule is behaving/creating different outcomes for different people. Please consider posting an image of your rule and an example entry from the audit log when this symptom occurs. That may provide more context for the community to offer ideas. Thanks!
