With the help of Zapier you will be able to integrate google forms and Jira to create issue in JIRA. Please find more details on below link.

Alternatively you can usr Jira Issue Collectors feature which  allows you to easily embed a Jira feedback form into your own web site. This form is typically accessed by clicking a 'trigger' tab exposed along the edge of pages in your web site.

I accidentally came across your question, even though it's an older one, I thought I could suggest an application that might still be beneficial for you or someone else.

My company uses this application: https://marketplace.atlassian.com/apps/1227085/smart-jira-forms?tab=overview&hosting=cloud  

The best part is that you won't need to deal with any integrations; you can configure everything directly within Jira. Simply create a form and customize it so that whenever a response is provided, it automatically generates a task in Jira.
