When a person is tagged in the comments by giving @ I can see the 2 usernames and email is shown in the drop down. Where i can see that there is only 1 user in the crowd and the jira user management.
Duplicate users means that both users exist in different user directories. As I understand your user db is crowd.
Please check if this user exist in your local jira db or other user db that you are using.

Is the user also added as a Customer in the JSM Project?
Or just available in the Crowd/Jira setup?

It's likely they're available in two locations.
