Using a form from automation jira cloud

Is it possible to launch a JSM form and once filled it creates an issue from automation for jira cloud?

I don't get your request.

Forms are used within JSM request types, based on creation of the request an issue is created in Jira.

I don't see where automation comes in to place?

The intention is that from a field, a form could be launched for the agent to fill it and create a new request.
