Linked field not populating correctly to summary

When filling out form that first field is linked to summary it is not grabbing data correctly

I don't know when changes were made but it use to be that either a summary box showed or you had to check this box and type in something random... IDK but I was able to uncheck it and save and 'Summary' did not show up in ticket submission
