I am trying to create a table within a form that the customer can edit, how is that possible?

I would like to create a table within a form allowing customers to input details of the product they wish to return that they have determined to be faulty.  I would then hope to add additional columns that can only be edited by the person the ticket is assigned to.

 

This is my first week with Jira and I am hoping it will assist greatly with a returns department processes and KPIs

 

I have now found that the table is in fact a skeleton and you must add individual fields to each box within the table. so after searching I managed to work it out myself.

@jeff I am also looking for an answer to this.  Did you ever find a solution?  I will keep investigating.  If I find how to do it, I will post here. 

Also looking for the same solution and also how to make those inputs required.
