How can attach a document/ documents as a knowledge base (Article) to a Jira form?

A user opens a ticket. How can add an Informative document to the form so that the user can use the information to fill up the request form?

Hi @Neda Ghavami 

Short answer is you can't. KB articles will appear as the user writes the summary of the request, or when he searches for something. WHat you could do is to change the description of a field, and point the user to the right link.
