Currently in the existing FORM version in the CLOUD env, it is not supported. It is being developed by Atlassian's FORM's team as one of their top priority. Take a look at this video - https://www.loom.com/share/ce04aee49faf4f45bcf24fea024b187f

As a work around, you will need to use Request Type configuration to include the "Attachment" field in the Request Type along with your FORM setup. However, the Attachment field will always be located before the FORM layout.

A quick update on this - you can now insert attachment fields into the form via Project Settings > Forms.
