Hi, does anyone have a good plugin for adding "How useful was this page?" at the bottom of each page? It would be extra nice to have a field you can have emailed to the administrator with feedback. I've searched around but haven't found anything that seems mature.

You could try out the Content Survey and Reporting Plugin, which was developed by some of Atlassian's support engineers to rate the content in our knowledge base articles on confluence.atlassian.com (for example, check out the Confluence Knowledge base).
The plugin has a few other features, in addition to content feedback, so it might be overkill for your needs, but it's still worth checking out. It's downloadable for free on plugins.atlassian.com
Caveat Emptor: although written by Atlassian employees, the plugin itself is unsupported. :-)

For the Confluence 4.0 EAP and Beta program, we created a simple "Got Feedback?" button that essentially is an iframe to a WuFoo form (which you can change yourself). We found this to be pretty sucessfull. You could easily take this and adapt it to your use case. Source is here.
