I wish to create many fields in a issue to hold a label plus an especific date

Example:

[Name1_Surname - 10/09/2021]
[Name2_Surname - 12/03/2021]
[Name3_Surname - 17/02/2021]

The only field that fits that off-the-shelf is a large text field, which literally would hold that as text.

I suspect you want this as probably a table, with name/surname/date fields, for which you will need an app to provide such a field (Table Grid is the one I look at first)

Jira doesn't have multiple-line custom fields where you add as many entries as you like.  Other than "comments" which is not a field anyway.
