Randy,
When you say Confluence Form, do you mean when you create an issue in Confluence using the Jira Issues Macro?
By default, when a user tries this, it will ask them to log into Jira in order to create the issue.
Can you let us know exactly what you're trying to do and what's the current behavior? what's your setup like? Is Jira and Confluence on the same server using the same URL but a different port and context path? Is the user base the same?
