This weekend you've released the new OnDemand header.
We're using the OnDemand Documentation theme for our spaces. Before the release once could toggle the sidebar in the header. Now this is gone.
This leaves some of our users in tricky situation. Some of them had it toggled off and can now no longer turn it on.
This issue is currently under investigation.
The button is there though, when you hover your mouse over the header just to the left of the search field, you will see it "light up"
The issue is that there are two different documentation themes included with Confluence OnDemand: one is called "Atlassian OnDemand Documentation Theme" and the other one simply "Documentation Theme". With the launch of the new header UI, the Atlassian OnDemand Documentation Theme should have been migrated to the standard Documentation Theme, but unfortunately this did not happen correctly.
We'll be fixing that in an upcoming deployment, but in the meantime you can fix it manually by switching your spaces to the plan Documentation Theme. You can change the theme by going to "Space Admin" in the Browse menu, choosing "Themes" in the "Look & Feel" section, then selecting "Documentation Theme".
You'll see a warning, "The theme you have selected does not include the Atlassian OnDemand navigation bar" but this warning is also a mistake that no longer applies with the new UI and will be removed. You can safely ignore it.
Once you've switched to the Documentation Theme, you will see the toggle button in the header and be able to drag the sidebar's right edge to resize it, just as before. Thanks again for pointing this out to us, and I apologize for the confusion.

The toggle icon is present in our instance on pages but disappears when viewing the associated attachments pages. Shouldn't it appear on both?

I would contact support if you have not done so, since it is related to the upgrade.
