The only free plugin I can think of that supplies that functionality is the Form Mail NG plugin by Adaptavist: https://marketplace.atlassian.com/plugins/com.adaptavist.confluence.formMailNG

It seems that it is only compatible with 4.2.1 and lower, but this would be your best option assuming that you're not running the latest version of Confluence. However, you can also check the following list and get an idea of the commercial plugins available for creating forms in Confluence, but please ensure that you check the version compatibility to make sure the plugins you install/purchase are compatible with your version of Confluence: https://marketplace.atlassian.com/search?application=confluence&q=form

Hi John and thanks for your answer.
I tested Form Mail NG on Confluence 4.3.1 and it doesn't work, it has a loop on the javascript as reported here
