Error while generating placeholder. Is the server available?

I use the menu-pick "Insert more content" --> "Table of Contents"

I use all the default settings and then click "Insert".

I get a small pop-up with the message: Error while generating placeholder. Is the server available".

What is this about and what do I need to tell my admin that needs to be fixed?

errors such as "Error while generating placeholder. Is the server available" could be related to an incorrect reverse proxy configuration in Confluence. 

Do you use proxy with your Confluence server? If yes, you need to follow the instructions on this page: https://confluence.atlassian.com/kb/securing-your-atlassian-applications-with-apache-using-ssl-838284349.html#SecuringyourAtlassianapplicationswithApacheusingSSL-PartA.ConfiguretheAtlassianapplications.

Make sure to add proxyName, proxyPort, secure, and scheme to Confluence’s server.xml and restart the server.
