This problem started about a week ago and it has never been a problem before
Now we nor our customer can approve any documents any longer as we are getting a GraphQL error when trying to transition from Review to Approved

Hope you are doing well. The error could be because:
The user might no longer have relevant permissions to transition the issue or one or more of the workflow post functions/validators set on that transition are failing.
Please check for any recent changes in the permissions or Workflow.

Yes it was the problem and we had to add the "Workflows for Confluence Cloud" user with "Can edit" privilege's in our overall external structure.
