Balloting Add In

Is there any add-in or tool in the suite that can be use for balloting?  Use case is that we want to notify space users about an issue or "ballot" and record their votes electronically with comments.

Hey Orlando, welcome to the Community!

I know there are a couple of survey addons available, I have personally used Forms for Confluence from Adaptavist, wasn't too hard to use.
