Auto unapprove Pull Requests add-on not working properly

"Bitbucket Server - Auto Unapprove Pull Requests" add-on sometimes doesn't auto unapprove when there was a change to the pull request. This issue happened on a pull request where another merge happened on that branch. This is a pattern we discovered, but might be something else. Appreciate your help or if you know there are certain cases where this add-on wont work?

Only changes to the source-branch that cause a material change to the diff will trigger the unapproval.  Clean sync-merges, amends, rebases, etc, do not trigger the unapprove.
