Why did changes to initiatives in Portfolio get attributed to the wrong user in the activity stream?

User A was making changes and updates in Portfolio to her own project initiatives and epics. Looking at the activity stream and the history of the issues, all activities were attributed to User B. This is scary. User B was not using Portfolio and is not in the same project.
