Re-opening bugs

Question

Dears, we are developing a website, we user JIRA to write user stories, bugs, ...

we have two weeks sprints, we put some user stories on each sprint, when the development team finish the sprint, the QA team tests the stories, any founded bug is being created on JIRA and related to its user story, so the user story will not be set as 'DONE', until all its related tasks and bugs are done.

Now, if we have a 'DONE' user story, and after couple sprints, a new bug is founded "accidentally" on that 'DONE' user story, should we create a new bug and relate it to the user story (that includes that we have to re-open the user story)? or what should we do?

but if we create a new bug and linked it to the previous user story, then the user story is re-opened, is that correct to re-open the user story? which maybe 'DONE' since 3 months ago?

It's better opening a new issue, linking it to the previous one as related.

Different "diaseases" can share some sympthoms, so it is better not to assume we are facing the same bug.
