Adding images has started to fail on the attach files panel, anyone know how to fix?

On both OSX and Window, Chrome and Safari tested. Both used to work yesterday. No known update to browsers in that time frame.

Two instances show up on the panel when only 1 image was added.

Suggestions welcome

thanks
