Welcome to Atlassian community.

You need to create a new service project, you can create a new one based on one of template offered by JSM.

If it's your first time in JSM, I encourage you to follow this tutorial: https://support.atlassian.com/jira-service-management-cloud/docs/get-started-with-jira-service-management/

IF you have setup a JSM project and created ac form, ac form needs to be linked to a request type, existing or new one created.
