For whom this app is intented to be used by? Customers or users? And kindly confirm that your need is something "Survey-ish". You want users to fill surveys using multiple pages.

Hi @Mahalakshmi Sivasamy, Previously, we used "Extension for Jira Service Management," but now we have started using native forms.

Hi @Mahalakshmi Sivasamy - You need to provide some more context around what you are actually trying to accomplish. What is your process? How would users interact with the form(s)?

Mahalakshmi Sivasamy

Hi @John Funk ,
We are developing a dynamic Jira Service Management form to streamline request submissions through the customer portal. This initiative aims to enhance user experience and improve usability, specifically designed for non-licensed users to submit their requests efficiently.

At this point we have a lengthy form that displays all the fields which is not easy for users to work with.

Hi @Manoj Kumar Gangwar , Thank you for your time on this, We are using JIRA native forms now. But we are looking for some extensive features on top of it.

Hi Alex,

Thsi app will be used by customers to submit the form. And form is not just a survey, it is a requirement submission form through jira service management.

Hi @Sebastian Krzewiński , Thank you for your time on this.

You can add conditional sections to JSM advanced forms so that fields are hidden and only displayed if there are certain values in other fields.

Is that what you are looking for?
