Advanced Form Submission - Automation not triggering

Question

Hi all.

I have created a form which, when submitted creates a User Story:

The User Story is created and contains the form which is correct. The Security Level is defaulted to the company level, which includes - Project Role (atlassian-addons-project-access).

I have also set up some automation that should append a couple of labels to the user story. Unfortunately the automation doesn't seem to work and I can't figure out why. Below is the Automation rule:

The Audit Log for this rule only shows Config Changes. There are no other statuses:

Can anyone see what I'm doing wrong here? I'm expecting a user story to be created when the form is submitted, and some labels added.

use the issue created trigger based on issue type story.

From what I can see from your post, I would guess the automation is not firing.  Can you share how you configured the trigger "when forms submitted"? And as @Marc -Devoteam- mentioned this is probably not the correct trigger.
