How to create an issue?

I've a Jira cloud company managed software project. I configured Change Request as one of the issue type and I was able to create issues as expected. However, I used the new feature "Forms" in Jira software; mapped the Change Request to the forms, it pulled all the fields correctly. However, now I can create a change request issue only via forms and not the regular way from "Create" button. Is it possible to create an issue via the regular way by "Create" and also by forms?

I'm assuming you're checking this in JSM, while using Forms if you've linked the form fields to a custom field in Jira then yes, adding those fields to the Create issue screen will allow you to create a issue with the same data, but if you've used Conditional statements in forms then those unfortunately don't work in regular create issue screens.
