I have cloned a initiative in Jira and selected "clone child issues" however once it cloned the initiative the child issues are missing.

If you are willing to try a third-party app, you can try our Deep Clone for Jira.
Deep Clone can clone whole issues hierarchies (Initiatives → Epics → Tasks → Subtasks) with its Epic/Tree Clone feature.
